Patient Notice – FMLA Paperwork Administrative Fee
At Therapeutic Village, PLLC, we are committed to assisting our patients with documentation needed for employment or medical leave purposes. Please note that the completion of Family and Medical Leave Act (FMLA) forms requires additional administrative time outside of your clinical appointment.
